Before You Arrive

There are a few things that we want all of our patients to know before they arrive for their initial appointments. The more you know ahead of time, the better prepared you can be. If you have any questions or concerns, please call our dental office; one of our team members will take their time explaining what you want to know.
Your First Visit
Our front desk team will warmly greet you and provide you with the appropriate forms if you haven’t already completed them. Once all the paperwork is complete, we’ll move to see you as soon as possible. The first visit will be based around learning more about your dental and medical history while becoming familiar with your mouth and the state of your oral health. Our findings during your first visit will set the tone for your treatment, and follow-up visits will be scheduled according to your needs.
Patient Forms
For added convenience, you can find all of the paperwork that you’ll need to complete for your initial appointment here on our website. You can just download them and print them out on your own time at home, then fill them in at your leisure. It can take several minutes to complete these forms; thus, if you bring them in already completed, you can shave several minutes off your waiting time, meaning you’ll be able to meet with our dentists that much faster.
